Commission becomes claimable only once the dentist outcome is
Treatment Complete and the payment status is Paid in full. It is worked out by the
system at 1% of the total treatment cost, so there is no formula to break.

This is where the dropdowns come from. Adding something here makes it
available to everyone straight away. Retiring something hides it from the dropdowns but leaves
every past lead exactly as it was — nothing is ever deleted.
