What is owed is counted by the date the treatment was finished. What has been paid is counted by the date the money went out — so a June treatment settled in July belongs to July's run, which is what the bank statement will say.
Commission is 1% of the treatment cost, worked out by the system. It becomes claimable once the dentist outcome is Treatment Complete, the payment status is Paid in full and the invoice number is filled in. From there it is claim → approve → pay: a coordinator claims the lines they want paying, a manager who is not paid by that claim signs it off, and accounts get a spreadsheet. The amount is frozen at the moment it is claimed, so a treatment cost edited afterwards cannot quietly change what accounts has already been told.
